*While we may honor the network rate for non-contracted plans  who otherwise cover  our services  via our participation with  an accepted network , we do not submit, process claims or  accept assignment, further terms or payment from out of network , non-contracted  plans, and /or subsidiary plans owned by accepted networks, government  insurance  or  TPA (3rd Party Administrator)  plans using our participating networks for  provider coverage, yet keep benefit info, and  process claims  & payment independently.     A detailed statement may be provided upon request after each visit for patient reimbursement efforts.
